Patient Rights:
- To receive care in a safe and clean environment, irrespective of age, gender, race, religion, nationality, sexual orientation, disability, or manner of payment.
- To experience considerate and respectful care that upholds personal dignity and is free from all forms of abuse, neglect, or harassment.
- To have their special preferences, personal values and beliefs, spiritual and cultural needs respected.
- To seek an additional opinion, within or outside the hospital, regarding clinical care without fear of compromising their treatment.
- To expect full protection of privacy and personal dignity during examinations, procedures, and treatments.
- To decide what information about their care is shared with family or others, and under what circumstances.
- To receive appropriate assessment and effective management of pain related to their medical condition.
- To receive respectful and compassionate care at the end of life.
- To have their health information kept confidential, except as required by law.
- To be informed by their doctor about the risks, benefits, alternatives, and expected outcomes of proposed care and treatment.
- To knowledgeably refuse any care, diagnostic procedure, or treatment after understanding the consequences of refusal.
- To give informed consent before undergoing blood transfusions, anesthesia, surgery, participation in research protocols, or any other invasive or high-risk procedures/treatments.
- To be informed about the process for voicing complaints, expressing concerns, or addressing grievances.
- To access their clinical records and health information.
- To be educated about their care plan, progress, and healthcare needs and to participate in care and treatment decisions.
- To know the identity and professional status of individuals providing their service.
- To be fully informed about the expected cost of their treatment.
- To be aware of the hospital’s rules and regulations.
Patient Responsibilities:
- To provide accurate and complete demographic information, including full name, age, address, telephone number, email ID, details of next-of-kin, and insurance company/TPA/employer.
- To share complete and accurate information about your health, including your medical history, current medications, past investigations, and any other relevant medical conditions.
- To actively participate in your treatment plan, follow the prescribed regimen, and keep your doctors and nurses informed about the effectiveness of your treatment.
- To attend scheduled appointments and notify the hospital if you need to reschedule or cancel.
- To respect the rights of other patients, visitors, and hospital staff, treating them with courtesy and dignity.
- To comply with hospital rules and safety regulations, including maintaining noise levels, adhering to the visitors policy, and avoiding weapons on hospital premises.
- To avoid bringing valuables to the hospital and to assume responsibility for the safety of your personal belongings.
- To adhere to the hospital’s no-smoking, no-chewing tobacco, and no-alcohol policies.
- To provide complete and accurate information to your insurance company to facilitate claims processing.
- To fulfill your financial obligations for healthcare as promptly as possible.
- To acknowledge that patients requiring emergency care may take priority.
- To respect the property of others and the hospital itself.
- To accept measures taken by the hospital to ensure your privacy and the confidentiality of your medical records.
- To communicate with healthcare staff if your condition worsens or deviates from the expected course of treatment.
- To understand all instructions before signing consent forms.
- To familiarize yourself with the 'Patient Rights' charter and seek clarification if needed.
